Responsibilities

  • Coach patients, particularly non-tech savvy older adults, through the onboarding process for the remote patient monitoring program
  • Provide assistance with the setup and use of remote monitoring hardware, ensuring patients can successfully use the devices
  • Conduct outreach via email and mobile communication (iPhone, etc.) to inform patients about the program and guide them through enrollment
  • Assist patients in scheduling calls with their clinic for in-person discussions about the program if needed
  • Maintain accurate records of patient interactions and onboarding progress using tools like ZenDesk, HubSpot, and G Suite
