As Manager, CHN, you will oversee social case management services, ensure quality and productivity, and participate in interdepartmental groups. We are looking for a passionate and committed individual with a Master's Degree in Social Work, LMSW certification, and 5+ years of experience.

Requirements

• Master's Degree in Social Work
• Certification: Licensed by the State of Texas as a Licensed Master Social Worker(LMSW) Required
• Bilingual in Spanish strongly preferred
• 5+ years of experience

Responsibilities

• Provides supervision of social case management services
• Provides coverage as needed to meet program goals
• Plan, develop, organize, direct implement and monitor a coordinated approach to social case management services
• The role of Manager, CHN responsibilities include overseeing the activities of the department staff, ensuring quality, productivity, functional excellence, and efficiency to accomplish strategic and operational objectives
• Participates in interdepartmental groups to represent social case management needs in different aspects of Imagine Pediatrics
• Establishes goals for performance standards
