Responsibilities

• Spearhead the operations of various care delivery pods, ensuring alignment with organizational objectives and values
• Drive initiatives to exceed growth targets, profitability goals, and clinical outcomes metrics
• Work closely with marketing, population health, and clinical teams to establish patient engagement and outcomes goals for each care delivery pod
• Foster a collaborative environment to achieve shared objectives
• Assume responsibility for pod performance, monitoring progress against established goals
• Utilize data-driven insights to identify areas of success and opportunities for improvement, ensuring adherence to high standards of quality care
• Analyze relevant data and metrics to gain comprehensive insights into pod performance
• Utilize findings to inform strategic decision-making and drive continuous improvement efforts
• Develop and implement plans to test new ideas and processes within care delivery pods
• Continuously seek opportunities to enhance performance and exceed goals through innovation and optimization
• Identify opportunities to streamline and optimize processes within care delivery pods
• Collaborate with cross-functional partners to implement improvements and provide constructive feedback
• Co-lead daily huddles for the clinical team alongside pod provider leads, fostering effective communication and collaboration
• Cultivate a supportive and inclusive work environment that encourages teamwork and professional growth
