Requirements

• Appropriate CNS, NP, PA licensure/certification in the state of Florida
• Compliant with state of Florida requirements for CEUs and licensure
• Experience in telehealth, wellness visits, and preventative care is required
• Excellent communication and interpersonal skills, especially in a telephonic setting
• Strong organizational skills and attention to detail for effective patient documentation
• Comfortable utilizing and managing electronic health record (EHR) systems, medical software/technologies to ensure accurate documentation, care coordination, and data security

Responsibilities

• Conduct telephonic annual wellness visits
• Identify patient needs, provide health education, and offer lifestyle recommendations to support preventive health measures
• Create patient care plans
• Document patient interactions and maintain accurate records in compliance with company policies and healthcare regulations
• Collaborate with the care team to ensure patients receive appropriate referrals and care
• Identify and enroll patients who would benefit from Chronic Care Management (CCM)
• Review ongoing CCM patient care plans
