Join Each's dynamic team as a Senior Stepped Care Coordinator in Queensland, contributing to the Better Connect program's integrated, person-centred care. This full-time, ongoing position offers hybrid/remote work options across various locations. Responsibilities include leading a team of Stepped Care Coordinators, overseeing care planning, building relationships with partners, providing training and support, and conducting audits and reports. The ideal candidate possesses expertise in mental health and AOD, leadership skills, a commitment to cultural safety, strong collaboration abilities, and critical thinking skills. Tertiary qualifications in a relevant field and a current driver's license are required. Requirements

  • A solid foundation in mental health services with comprehensive knowledge of relevant systems and legislation
  • Experience leading teams, ensuring strong clinical governance, and upholding high standards of care
  • A deep commitment to cultural safety and applying trauma-informed, person-centred practices
  • Exceptional interpersonal skills with a proven ability to build and maintain professional relationships across multiple services and disciplines
  • Strong analytical skills to address complex challenges and deliver effective solutions for client care
  • Tertiary qualifications in Social Work, Health and/or Welfare related sector
  • Current state-based driver’s license

Responsibilities

  • Provide leadership and day-to-day oversight of a small team of Stepped Care Coordinators, supporting their professional development and ensuring quality service delivery
  • Support & oversee care planning, transitions, and team reviews
  • Maintain strong connections with hub partners and service pathways
  • Collaborate with the Better Connect lived experience team for community responses
  • Provide training, supervision and support in screening and assessing complex clients
  • Guide on mental health referral pathways
  • Offer high-quality engagement in AOD, mental health, and suicide prevention services
  • Ensure continuity of care with Better Connect providers
  • Perform audits and submit reports for quality assurance and training needs

A maintained relevant annual registration and/or membership of professional body
