Requirements

  • Minimum of one year of experience in navigating social services as an individual, case manager, social worker, or care navigator worked at community based org, knows the landscape
  • Empathy and passion for helping vulnerable populations
  • Quick learner who is excited to become an expert in new technologies
  • Flexible, meeting the needs of the dept. Shift priorities/balance/bounce
  • Ability to set priorities and work independently while following standard operations procedures
  • Friendly and compassionate; comfortable communicating directly with clients in need and community-based organizations
  • Resourceful and fast-paced; able to conduct quick, exhaustive online research

Responsibilities

  • Manage a caseload of customers' clients that averages 50 referrals completed weekly. This will comprise initial calls, assessments, follow-up, and closing the loop with partner organizations
  • Utilize the Unite Us Platform to facilitate seamless care coordination services for customers, ensuring contractual requirements are met
  • Conduct intake and needs assessment screenings for clients seeking assistance to understand their needs and preferences for care coordination services
  • Collaborate with partner organizations to identify and connect clients with the appropriate services based on their needs, eligibility, and geographic preference
  • Serve as a liaison between clients and service providers to ensure effective communication and coordination of care
  • Conduct follow-up with clients and service providers to ensure receipt of services/benefits
  • Under the social services landscape and provide supportive feedback to community-based-organizations serving Care Coordination clients to ensure network standards are met
  • Continuously expand knowledge of community resources, services, and programs to understand community issues and build relationships with other users of the Unite Us platform
  • Be a champion of customer-centric culture and deliver excellence in customer experience by meeting and exceeding service expectations and solutioning workflows that drive impact
  • May support special projects and backend workflows that help drive success for customers, communities, and consumers

Preferred Qualifications

  • A degree in social work or a relevant field
  • Clinical credentials
  • Case mgmt/Care Coord cert/CHW
