Summary
Join Sprinter Health as a Patient Care Coordinator and play a vital role in expanding access to healthcare. You will manage patient schedules, ensuring appointments have necessary information and documentation. Collaborate with healthcare systems and plans to obtain missing information and provide updates. Validate provider contact information for timely result sharing and compile medical records into visit summaries. Work closely with the clinical manager and escalate issues to senior coordinators. This role requires strong organizational and communication skills, experience in a primary care setting, and proficiency with EHR systems.
Requirements
- At least 2 years of previous experience working in a primary care setting / health care administrative environment
- Proficiency in using electronic health record (EHR) systems and other healthcare software
- Experience coordinating with national labs such as Quest or Labcorp. This can include but not limited to requisition review, issue investigation, result tracking
- Excellent organizational skills and attention to detail to ensure accuracy in appointment documentation and record-keeping
- Strong communication and interpersonal skills, with the ability to effectively liaise between clinical and administrative teams
- Ability to adapt to changing priorities and work effectively in a fast-paced environment
- Commitment to maintaining patient confidentiality and delivering high-quality, patient-centered care
Responsibilities
- Review the patient schedule to ensure all next-day appointments have the necessary registration information and documentation
- Work closely with our health system and health plan counterparts to obtain any missing appointment information, provide appointment updates
- Validate provider contact information to ensure results can be shared back with the provider in a timely manner
- Review all appointments after completion and compile relevant medical records into a unified visit summary to be shared with providers
- Work closely with our clinical manager to review appointment progress
- Escalate questions and concerns to Care Coordinators II as different patient situations arise
