Responsibilities

• Identify and qualify prospective patients who could benefit from Carda Health's remote pulmonary care program through various channels, including inbound inquiries, referrals, and outreach efforts
• Educate patients on the features, benefits, and value proposition of Carda Health's telehealth services, highlighting the convenience, accessibility, and effectiveness of remote pulmonary care
• Conduct thorough assessments of patient needs, medical history, and eligibility criteria to determine suitability for enrollment in the program
• Guide patients through the enrollment process, providing personalized support, answering questions, and addressing concerns to ensure a seamless and positive experience
• Collaborate closely with internal teams, including patient care coordinators, healthcare providers, and technology specialists, to facilitate smooth transitions and continuity of care for enrolled patients
