Summary
Join Aledade as a Practice Transformation Specialist (PTS) and significantly impact patient care by assisting primary care practices in transitioning to value-based care. As a PTS, you will frequently visit assigned practices, acting as a project manager for various initiatives aimed at improving care and reducing costs. You will partner with practices to implement Aledade technology, train staff, and track progress toward goals. This role requires a strong understanding of the healthcare delivery system, clinical and administrative experience, and exceptional change management skills. Requirements
- Resident of Washington or Oregon and willing to travel to practices up to 20% in these states
- Graduate of an accredited college or university (4-6 years of significant and relevant work experience in medical practice management in lieu of educational requirements may be accepted, particularly with significant administrative experience in a clinic setting)
- 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes
Responsibilities
- Project management
- Exporting data and running reports
- Problem solving for efficient and effective progression of initiatives
- Identifying and developing suggestions for improvement plans
- Partner with Medical Director & Executive Director on project-specific timelines
- Communicate areas of concern, needed tools, or barriers to achieving goals
- Share best practices with teammates across Aledade
- Assist in the preparation of proposals, reports, updates, and summaries
- Identify opportunities for improvement in the operations
Preferred Qualifications
- Excellent oral and written communication skills (i.e., public presentation skills), organizational and project management skills, and team-building skills
- Graduate of an accredited college or university in nursing, social work, health technology or information technology-related programs
- Familiarity with process mapping and work-flow analysis tools
- Excellent computer skills and willingness to learn additional software applications
