Responsibilities

  • Engage with patients through consultations via phone or video to explain program expectations, check insurance coverage and schedule patient sessions
  • Utilize technology to document interactions, track tasks, and communicate effectively with the counseling team
  • Execute and optimize scheduling and care coordination to ensure positive patient experience and clinical team efficiencies
  • Ensure timely follow-up on care coordination needs
  • Diligent follow-up with patients to ensure appointment attendance and follow-up
