Remote Clinical Care Navigator

Summary

Join Lyra Health as a Clinical Care Navigator to manage crises, provide phone support, and connect clients to high-quality mental health care. This role requires excellent communication skills, crisis management experience, and a growth mindset.

Requirements

• Master’s degree in clinical social work or equivalent; LCSW or equivalent independent clinical licensure is required (e.g. LMFT, LPC, LPCC, etc)
• Must have two years psychiatric intake, assessment RB, other related experience
• Must have Two years post license experience (EAP/clinical call center experience preferred)
• Significant experience managing high-risk clinical situations (e.g., suicidality, mandated reporting)

Responsibilities

• Using evidence-based interventions, skillfully manage client crises, providing brief risk assessment, safety planning, and de-escalation with the goal of connecting the client to treatment
• Identify roadblocks to clinical care options for our clients using Lyra’s proprietary software and other methods to connect clients to the ideal treatment, based on specific needs and preferences
• Independently manage a caseload of complex, multi-need clients
