Requirements
- Be comfortable speaking on the phone and communicating via email with a diverse range of clients and providers
- Be excited about communicating efficiently, clearly, and comprehensively
- Be knowledgeable or interested in learning about complex healthcare systems
- Be detail-oriented and excited about problem-solving to get the job done
- Be excited about navigating a growing, rapidly changing, and sometimes ambiguous environment, which includes high-acuity mental health cases
- Have exposure to clinical information pertaining to client level of care, including high acuity cases
Responsibilities
- Coordinate outbound referrals based on clinical information, client's insurance coverage, referral source, and level of care
- Partner with health system representatives and providers to ensure clientβs next steps of care
- Work through ambiguity to establish and document ongoing Care Navigation processes whenever possible
- Represent Two Chairs on the ground with some of our closest partners
- Support clients who receive outbound referrals from Two Chairs via phone and email
- Serve as an empathetic point of contact as clients navigate the healthcare system
- Provide clients with the necessary contact information and scripting that they can use to advocate for themselves
- Answer any questions for clinicians who submit outbound referrals
- Escalate cases as needed and collaborate with clinicians on more challenging cases
- Collaborate with internal and external stakeholders on internal and external clinical referral process, client growth, and business needs
Preferred Qualifications
- Be empathetic and passionate about accessible mental healthcare - exposure to clinical settings is a plus, but not required
- Be Spanish speaking
