Clinical Care Navigator

Summary

The job description is for a Care Navigator at Lyra Health, a mental health care company that provides digital care platforms and global provider networks. The role involves managing crises, providing telephone support to clients with complex behavioral health issues, ensuring client safety, and connecting clients to high-quality, evidence-based providers and facilities.

Requirements

• Master’s degree in clinical social work or equivalent
• LCSW or equivalent independent clinical licensure is required (e.g. LMFT, LPC, LPCC, etc)
• Two years post license experience (EAP/clinical call center experience preferred)
• Significant experience managing high-risk clinical situations (e.g., suicidality, mandated reporting)

Responsibilities

• Using evidence-based interventions, skillfully manage client crises, providing brief risk assessment, safety planning, and de-escalation with the goal of connecting the client to treatment
• Identify roadblocks to clinical care options for our clients using Lyra’s proprietary software and other methods to connect clients to the ideal treatment, based on specific needs and preferences
• Independently manage a caseload of complex, multi-need clients

Preferred Qualifications

Bilingual, Spanish-speaking candidates strongly preferred (but not required)
